Annual reporting
For any subsurface user the end of the year is sharply associated with the need to provide an annual report to the supervisory authorities. First of all, this is reporting on forms 5‑GR, 6‑GR, 7‑GR, which is accepted in paper and electronic form by the Ukrainian Geological Survey to optimize the accounting of mineral reserves. Text blocks of all forms are filled exclusively in the state language, signed directly by subsoil users and must be certified with a seal. The forms are submitted in strict accordance with the requirements of Order No. 97, “On approval of reporting forms for accounting of mineral reserves and instructions for their completion,” issued by the Ministry of Ecology and Natural Resources of Ukraine.
Form 5‑GR reports on the annual production of raw materials at enterprises that extract solid fuels, metallic and non-metallic minerals. It is compiled separately for each type of raw material produced and submitted by February 5. It must be accompanied by an explanatory note that describes the logic of the form and, if necessary, specifies any unaccounted volumes of production for the previous year. Form 6‑GR reports on the annual production of raw materials at enterprises that extract oil, gas, condensate, propane, and similar valuable minerals. The form shows the write-off of reserves or their transfer to other groups. Form 6‑GR is compiled separately by each mineral user on their own site in cases where several enterprises operate simultaneously based on special permits in the deposits. Similar to Form 5‑GR, the document package is accompanied by an explanatory note and must be submitted no later than February 5.
Form 7‑GP reports on the annual extraction of groundwater. The detailed instructions for completing the form were approved by the order of the Ministry of Ecology and Natural Resources of Ukraine No. 97 dated March 14, 2016. The deadline for submission is no later than January 20. The main purpose of submitting this form is to optimize the accounting of groundwater reserves, collect information on their quantitative and qualitative characteristics, degree of geological and technical exploration, as well as losses of groundwater.
The report on the implementation of the work program, which is a mandatory appendix to the Agreement on Subsoil Use, must also be submitted to the State Service of Geology and Subsoil of Ukraine annually. The deadline for submission is January 31. The report form is available at the link.
